What does ratchet mean?

A 'ratchet' is a type of wheel with teeth around the edges with a tooth that only allows it to turn in one direction. This is a picture of a winch, used to move heavy objects like a car.
A 'ratchet wrench' only turns in one direction so you don't have to lift and move it to turn on every stroke.

You can also 'ratchet up the pressure,' for example, which would mean to escalate a situation by degrees.
